Kingsbury Family Medical Centre will have a small supply of Pfizer and Moderna vaccine available intermittently - please call to book appointment *on order as of October 7, 2021*

The Province of Manitoba has outlined eligibility criteria in order to receive this vaccine.  Eligibility criteria will frequently change therefore we encourage you to visit the Doctors Manitoba site to determine if you are eligible at this time.  You can also sign up to receive a notification for when you are eligible.

CONSENT PAPERWORK

If you require enhanced consent your provider will go over this at your vaccine appointment.  Links below are to consent forms if needed for Super site bookings.

Currently every Manitoban age 12 + is eligible to book for their COVID vaccine at Super-sites and Pop-up clinics.  Please click here for information from the government on how to book your vaccine.  You can book online or by phoning 1-844-626-8222 (1-844-MAN-VACC).

If you have any questions about receiving the vaccine you can book an appointment with your own MD for discussion.

IMPORTANT: If you require enhanced consent it will be reviewed with you by a trained provider at a vaccine supersite.  You can also call your own MD if you have any questions.

Enhanced consent is required if you are immunosuppressed, have an autoimmune condition or if you are pregnant/breastfeeding.
